Reborn Health Club hecht grote waarde aan de bescherming van uw persoonsgegevens. In deze verklaring leggen wij uit hoe wij omgaan met uw gegevens in onze rol als zorgaanbieder (diëtiek), sportfaciliteit en coachingsinstituut, met specifieke aandacht voor moderne verwerkingstechnieken zoals AI.

1. Persoonsgegevens die wij verwerken

Reborn Health Club verwerkt uw persoonsgegevens omdat u gebruikmaakt van onze diensten en/of omdat u deze zelf aan ons verstrekt. Wij verwerken:

  • Basisgegevens: Voor- en achternaam, adres, telefoonnummer, e-mailadres.

  • Bijzondere persoonsgegevens: Medische gegevens (via de diëtist/fysiotherapeut), gewicht, vetpercentage, fysieke belastbaarheid, blessurehistorie en medicijngebruik.

  • Digitale gegevens: IP-adres, websitebezoek-gedrag, video- en audio-opnames (na expliciete toestemming).

2. Waarom wij gegevens nodig hebben

Wij verwerken uw gegevens voor de volgende doelen:

  • Medische behandeling: Het voeren van een wettelijk verplicht medisch dossier (WGBO) voor diëtiek.

  • Uitvoering overeenkomst: Het opstellen van persoonlijke trainings- en voedingsschema’s.

  • AI-gestuurde optimalisatie: Het analyseren van consulten en trainingen middels AI-technologie om de kwaliteit van onze adviezen te verhogen en efficiënte samenvattingen te genereren.

  • Communicatie: Voor contact over afspraken en facturatie.

3. AI-verwerking en Innovatie

Reborn Health Club gebruikt AI als hefboom voor betere zorg en coaching:

  • Consult-analyse: Met uw toestemming kunnen audio- of video-opnames van intakes worden geanalyseerd door AI-software. Dit dient voor het vastleggen van de juiste behandelstijl en het genereren van hoogwaardige samenvattingen voor uw dossier.

  • Beveiliging: Wij gebruiken uitsluitend AI-tools die GDPR/AVG-compliant zijn en waarbij data niet wordt gebruikt voor het trainen van publieke modellen. Waar mogelijk wordt data gepseudonimiseerd.

4. Hoe lang wij gegevens bewaren

  • Medische gegevens: Conform de wet (WGBO) bewaren wij medische dossiers minimaal 20 jaar.

  • Algemene gegevens: Indien geen overeenkomst tot stand komt, worden uw gegevens na één jaar verwijderd.

  • Opnames: Ruwe audio/video-opnames voor AI-analyse worden na verwerking en dossiervorming binnen 30 dagen verwijderd, tenzij expliciet langer bewaard voor scholingsdoeleinden.

5. Delen met derden

Wij verstrekken uw gegevens uitsluitend aan derden als dit nodig is voor uw traject. Dit omvat:

  • Samenwerkingspartners: Zoals Optifit Fysiotherapie (indien relevant voor uw herstel).

  • Softwareleveranciers: (Bijv. Virtuagym, AI-tools) waarmee strikte verwerkersovereenkomsten zijn afgesloten.

  • Artsen: Rapportage aan uw (huis)arts gebeurt alleen conform richtlijnen, tenzij u bezwaar maakt.

6. Beveiliging

Reborn Health Club neemt de bescherming van uw gegevens serieus. Wij maken gebruik van:

  • SSL-certificaten voor veilige data-overdracht.

  • Beveiligde, medische cloudomgevingen voor dossieropslag.

  • Strikte toegangscontrole voor personeel.

7. Uw rechten

U heeft het recht om uw persoonsgegevens in te zien, te corrigeren of te verwijderen (met inachtneming van de wettelijke bewaarplicht voor medische dossiers). Verzoeken kunnen naar: info@rebornhealthclub.nl.

Reborn Health Club attaches great importance to the protection of your personal data. In this statement, we explain how we handle your information in our role as a healthcare provider (dietetics), sports facility, and coaching institute, with specific attention to modern processing techniques such as Artificial Intelligence (AI).

1. Personal Data We Process

Reborn Health Club processes your personal data because you use our services and/or because you provide them to us yourself. We process:

  • Basic Information: First and last name, address, phone number, email address.

  • Sensitive Personal Data: Medical data (via the dietician/physiotherapist), weight, body fat percentage, physical capacity, injury history, and medication use.

  • Digital Data: IP address, website browsing behavior, video and audio recordings (only after explicit consent).

2. Purpose of Data Processing

We process your data for the following purposes:

  • Medical Treatment: Maintaining a legally required medical file (under the Dutch WGBO Act) for dietetics.

  • Execution of Agreement: Creating personalized training and nutrition plans.

  • AI-Driven Optimization: Analyzing consultations and training sessions through AI technology to increase the quality of our advice and generate efficient summaries for your records.

  • Communication: For contacting you regarding appointments and invoicing.

3. AI Processing and Innovation

Reborn Health Club utilizes AI as a leverage tool for superior care and coaching:

  • Consultation Analysis: With your consent, audio or video recordings of intakes may be analyzed by AI software. This serves to capture the correct coaching style and generate high-quality summaries for your medical file.

  • Security: We exclusively use AI tools that are GDPR compliant, ensuring that data is not used to train public models. Data is pseudonymized whenever possible.

4. Data Retention Periods

  • Medical Data: In accordance with Dutch law (WGBO), we retain medical files for a minimum of 20 years.

  • General Data: If no agreement is established, your data will be deleted after one year.

  • Recordings: Raw audio/video recordings used for AI analysis are deleted within 30 days after processing and filing, unless explicitly kept longer for professional training purposes.

5. Sharing with Third Parties

We only provide your data to third parties if necessary for your trajectory. This includes:

  • Partners: Such as Optifit Physiotherapy (if relevant to your recovery).

  • Software Providers: (e.g., Virtuagym, AI tools) with whom strict data processing agreements (DPAs) have been concluded.

  • Physicians: Reporting to your GP only occurs according to professional guidelines, unless you explicitly object.

6. Security

Reborn Health Club takes the protection of your data seriously. We use:

  • SSL certificates for secure data transfer.

  • Secure, medical-grade cloud environments for file storage.

  • Strict access control for staff.

7. Your Rights
